Assesses patient's physical and psychosocial condition, develops a case management plan of care than includes efficient use of resources for the best patient outcomes. The plan of care is developed and modified under the direction of the physician. Documentation is completed in a timely manner and is done according to policy and procedure to meet government regulation and Regulatory Agency standards. Home care visits are made following the plan of care to patients of all ages including: neonate; pediatric; adolescent; adult; and geriatric; the majority of patients are over 65 years of age. The RN supervises care provided by Home Health Aides.
Principle Duties and Responsibilities:
Assesses and evaluates the patient on admission and a plan of care is developed based on the data collected using standards of evidence based practice that includes the efficient use of resources for the best patient outcomes. Develops and modifies the plan of care considering the diagnosis, patient history, physical and psychosocial status and the problem that are identified from the evaluation of the patient. For Home Health Care completes OASIS according to the timelines of Medicare regulation. For Hospice the assessment at start of care and the plan of care are completed and reviewed by the Interdisciplinary Group according to Medicare regulation. Develops and modifies the Home Health Aide plan of care and supervises the aide according to Medicare regulation. Determines aide competency for medication aide tasks. Documentation is performed accurately and appropriately and in a timely manner. Schedules visits and performs patient care activities in a time efficient manner to maintain productivity standards. Performs case management according to current standards balancing clinical and financial outcomes. Utilizes the nursing process and performs procedures and treatments according to policy and procedural guidelines and competency standards. Areas of skill includes but ore not limited to : I.V. infusion; tracheotomy care; stormy care; urinary catheter; wound care; pain management and system control; and patient education. Performs and coordinates patient care in a manner that promotes patient satisfaction by coordinating appropriate services for the patient in a competent. Schedules visits for patients according to the plan of care and responds appropriately to emergent needs as identified from patients, families, and the physician. Communicates appropriately and in a timely manner with physicians and other community agencies as needed. Works on-call on a rotating basis in evening, weekends and holidays. Takes referral information, makes decision to admit or schedule admit or decline. Responds to patient needs by making unscheduled visits. Communicates to Case Manager RN regarding visits and interventions. Complies with the standards of excellence set forth by Saint Francis Medical Center that includes: attitude; appearance; communication; commitment to co-workers; customer waiting; privacy; safety awareness; and sense of ownership. Assumes responsibility for professional growth of self and others through continuing education and sharing information and skills with others. Maintains knowledge of current trends and development in the field by reading books, journals and other literature and by attending related seminars and conferences.
Education:
Knowledge of nursing theory and practice at a level normally acquired through completion of an accredited School of Nursing.
Experience:
Minimum of one year of acute care experience required.Licensure: Current State license in Nebraska as a Registered Nurse required Must have and maintain at all times, a valid Nebraska driver's license and a good driving record. Must be able to utilize own transportation during work time.
