In collaboration with the Registered Nurse and the interdisciplinary team, including the physician to participate in development of the plan of care that includes efficient use of resources for the best patient outcomes. Documentation is completed in a timely manner and is done according to policy and regulation to meet government regulation and Regulatory Agency standards. Home care visits are made following the plan of care to patients of all ages including: neonate; pediatric; adolescent; adult; and geriatric; the majority of patients are over 65 years of age
Essential Functions:
In collaboration with Home Care Team, schedules visits and performs patient care activities in a time efficient manner to maintain productivity standards. Performs patient care according to current standards balancing clinical, quality and financial outcomes. Documentation is performed accurately and appropriately and in accordance with regulations and polices.
Utilizes the nursing process and performs procedures and treatments according to policy and procedural guidelines and competency standards. Areas of skill includes but or not limited to: Observation and reporting of patient condition, direct skilled services including stoma care; urinary catheter; wound care; pain management and symptom control; training and/or education of the patient and family.
Performs and coordinates patient care in a manner that promotes patient satisfaction.
Schedules visits for patients according to the plan of care and responds appropriately to emergent needs as identified from patients, families, and the physician. Communicates appropriately and in a timely manner with Registered Nurse, Home Care Team and other community agencies as needed.
Works on a rotating basis in evening, weekends and holidays. Takes referral information and relay to a Registered Nurse. Responds to patient needs by making unscheduled visits as directed by a Registered Nurse. Communicates to Home Care Team regarding visits and interventions.
Assumes responsibility for professional growth of self and others through continuing education and sharing information and skills with others. Maintains knowledge of current trends and development in the field by reading books, journals and other literature and by attending related seminars and conferences.
Qualifications
Education:
Graduate of an accredited School of Practical Nursing.
Experience:
One year of Licensed Practical Nursing experience required.
Previous home care experience preferred.
Must be able to communicate effectively in a variety of situations.
Requires the ability to function independently and to work with patients and families in their homes and within the parameters of their chosen lifestyles.
Licensure/Certification:
Licensed to practice as a Licensed Practical Nurse in the State of Nebraska.
Must have a valid Nebraska Drivers' License and a driving record compatible with the requirements of Saint Elizabeth Regional Medical Center.
Current Basic Life Support certification.
