This position works under the supervision of the Team Coordinator and reports to the Director and/or Management Council. The Nursing Assistant provides support for the direct management of the patient’s environment and activities of daily living under the direct supervision of the Registered Nurse

The Nursing Assistant II also performs general clerical duties including contributing to maintenance of the patient record and accurate patient charges.

Essential Job Functions:
Participates in patient’s immediate environment.

Participates in patient nutrition.

Prepares patient rooms for admission.

Participates in patient’s activities of daily living and provides clinical support within level of preparation.

Demonstrates the knowledge and skills necessary to provide care based on the physical, psychosocial, safety and related criteria appropriate to the age of the patients served.

Works directly with patient in planning and implementing patient’s activities of daily living including but not limited to: bathing, hygiene, ambulation, transporting, pre- and post-op care (including surgical preps), and dismissal functions to promote and maintain health.

Adheres to Nursing Structure and Process Standards and the patient’s plan of care in providing patient care.

Performs selected proc! edures and treatments.

Orients the patient and family to the room, department, policies, and schedules.

Assists the Nurse to maintain observations of patients by anticipating and responding to patient needs and processing call lights.

Ensures that the Nurse remains informed of patient status and activities.

Documents routine elements of care in the medical record.

Collects and documents patient data.

Maintains patient confidentiality.

Participates in the patient discharge as directed by the physician.

Customer Service:
Receives, greets and directs patients, family members, clients, visitors and physicians in a positive and conscientious manner adhering to our Embrace the Spirit standards and behaviors. Problem-solve, offer and give assistance as needed.

Additional Essential Job Functions for Level II:
Maintains patient's medical records

.

Coordinates admission communication process including bed placement, admit discharge transfer, death data and physician personal order sets.

Requests medical record from Health Information Management.

Maintains patient chart with appropriate orders, progress notes, and nursing forms. Makes sure all forms within the patient's chart are labeled with the correct patient information.

Accurately and timely files and purges patient reports as they are received from other departments according to the prescribed process.

Obtains a signed release of information form from the patient, when requesting patient information from a physic! ian's office or another medical facility. Faxes the release form to Health Information Management and the original form is to be placed in the chart under the Admin tab.

Makes appropriate copies of current chart to go to the receiving facility or agency when a patient is transferred to another facility or home health agency.

Sends copy of the patient transfer form and discharge med list to the appropriate physicians.

Tears down patient dismissal charts according to universal procedures. Places the dismissal chart along with the patient's old medical record in the Health Information basket.

Obtains daily patient computer reports from Data Processing or the printer. Removes temporary reports and places current reports on the patient's chart.

Thins medical record charts according to universal procedures.

Accurately and expeditiously processes physician orders in Meditech. Faxing all orders containing medications to the phar! macy department. Calls appropriate departments for STAT testing.
Responsible for patient charges.

Compares the current patient list with the computer census at the beginning of the shift. Maintains an accurate log of the nursing unit's daily admissions, dismissals, and transfers. Compiles new unit 24-hour census sheet daily.

Maintains SDC Observation Log Sheet. Enters patient's daily equipment and SDC and Observation charges into Meditech.

Reviews patient's length of stay with the logbook and charge report insuring the patient is being charged appropriately and notifying patient accounting of any discrepancies.

Calls medical supply companies to obtain or discontinue equipment for patient's current needs such as Brooke shoes, profess, and specialty care beds.

Participates in the charge audit process.

General Clerical/Secretarial responsibilities.

Serves as the communication coordinator of the nursing unit. Answers unit telephone and patient call lights. Relays messages to a! ppropriate staff members.

Greets physicians/patients/visitors and answers their questions; directs them to areas within the Medical Center.

Calls the CARE Office to set up outpatient testing upon discharge.

Maintains an inventory of current patient chart forms used within the Medical Center. Maintains a supply of patient chart forms and packets on the unit and current physician order sets.

Maintains and orders clerical supplies for the nursing unit.

Orders medical supplies for the nursing unit such as blood pressure cuffs, batteries, patient room phones, spectra link phones, and other equipment as needed.

Sorts and distributes unit's mail to appropriate staff members' mailboxes.

Compiles the unit daily costs per unit of service for the Director.

Staples patient care summary reports and nursing report sheets.

Places nursing assignments and updates patient information in the composer system. Ente! r spectra link numbers and shift assignments into Meditech. Place shift! assignment and spectra link number on assignment board.

Keeps the patient label supplies adequate and updated.

Performs other duties as assigned by Director/Team Coordinator.

Serves as a resource person to other staff members.

Qualifications

Education:
High school graduate or equivalent required.

Medical Terminology preferred.

Approved Certified Nurse Assistant Course (CNA).

Approved training for clerical functions if performing marginal job functions that are clerical in nature.

Licensure/Certification:
Currently on the Nebraska Nurse Aide Registry.

Currently on the Medication Aide Registry (only as related to administration of oxygen) or obtain training, complete competency and be registered on the Medication Aide Registry within 30 days of hire.

Maintain Medication Aide Registry stat! us with demonstrated competency every two years.

Valid Basic Life Support (BLS) certification required.

Maintain CNA certification including 12 continuing education hours (720 minutes) per year required.

Current students taking health-related course of study will meet the continuing education requirement with successful completion of at least 3 credit hours.

Courses in LEARN will count towards meeting this requirement, including but not limited to Rapid Regs and Corporate Compliance courses.

It is the associate’s responsibility to provide documentation of the education required.

Experience/Pre-Requisites:
Prior Nurse Assistant experience preferred.

Must be able to work from detailed verbal or written instructions and uses judgment for routine situations.
