Ensures safe, accurate and appropriate use of medications, by evaluating prescribersâ orders for dosing, allergies and contraindications. Supervises the work of suppor! tive pharmacy personnel. Answers pharmacy-related questions for other health professionals and the general public. Performs pharmacokinetic monitoring and consultations, reports adverse drug events and variances involving medications. Recommends appropriate and cost-effective drug therapy options. Provides medication counseling to patients and their families. Actively participates and facilitates the medication reconciliation process upon admission, transfer, and discharge of patients. Promotes and participates in the defined missions of the hospital and the pharmacy. Actively promotes the profession of pharmacy by participating in the education of students on clerkships and experiential experiences.

Appropriately reviews medication therapy and intervenes with healthcare team as warranted.
Evaluates medication order for appropriate medication selection, dosage, route, frequency, dura! tion of therapy and drug interactions (drug-drug, allergy, dru! g-disease, drug-lab, drug-food) in relation to the patientâs clinical status.
Performs daily profile review to assess appropriateness of medication therapy as detailed in âaâ in relation to patientâs ongoing and changing disease state and clinical status.
Appropriately and routinely utilizes formulary agents; follows appropriate procedures for the procurement of non-formulary items and the use of patientâs own medications.
Monitors pharmacokinetic patient profiles in accordance with policy. Performs accurate and timely pharmacokinetic consults.
Demonstrates competency entering medication orders into SMM or verifying a CPOE order as appropriate.

Responsible for the timely and accurate dispensing of pharmaceuticals, ensuring proper preparation, labeling, dispensing and billing.
Reviews medication orders and appropriately triages urgent, routine, and PRN medication orders
Understands and demo! nstrates appropriate order entry, preparation and documentation of the following pharmaceutical products: Controlled substances, Chemotherapy, TPN solution, IV solutions, Continuous infusions, Injectables, Oral preparations, Respiratory care items, Home medications, Non-formulary medications and Investigational Medications.
Provides appropriate supervision and timely checking of pharmaceutical products prepared by technicians and interns.
Proficient in describing appropriate delivery of all medications including routine meds, controlled substances, and chemotherapy.
Effectively identifies and resolves routine professional practice problems on a regular basis using critical evaluation skills.

KNOWLEDGE, SKILLS AND ABILITIES:
Mathematical skills necessary for algebraic conversion, subtractions, multiplication, and division.

Previous experience in health care area or unit dose medication preparation helpful but not required.
EDUCATION AND EXPERIENCE:
Bachelorâs degree from an accredited college or university in pharmacy required
Minimum 1 year experience in a pediatric hospital pharmacy or pediatric homecare pharmacy setting preferred
CERTIFICATIONS/LICENSURE REQUIREMENTS:
Current and Valid Nebraska pharmacist or be eligible for licensure in the state of Nebraska required.
