Under the supervision of the Director of Patient Care Services coordinat! es all care for the trauma patient. Promotes optimal trauma care and provides leadership and facilitations to promote improved acute stroke patient outcomes in collaboration with nursing, medical staff, ancillary departments, and pre-hospital providers. Incorporates research, performance improvement, administrative, educational, and clinical activities to enhance care of the trauma patient within the system as well as with the patient and family.
Cares for patients with a variety of medical problems aged neonate, pediatric, adolescent, adult and geriatric at various developmental levels. Patients are from both traditional and non-traditional family structures with a variety of sociocultural and educational backgrounds.
Essential Duties:
Responsible and accountable to be aware of all trauma and acute stroke patients admitted to the hospital. Facilitates the coordination and delivery of trauma care and acute stroke care from resuscitation through ! rehabilitation; promotes a seamless course of stay in the cont! inuum of care.
Assess trauma patientâs mechanisms of injury for appropriateness of management (utilization of trauma system).
Reviews all trauma team activations and stroke team activations.
Keeps Trauma Medical Director informed of trauma patients in-house and issues that need immediate attention. Keeps Stroke Medical Director informed of acute stroke patients in-house and follows patients with stroke during hospitalization for the purpose of applying evidence-based practice to improve patient outcomes.
Develops a collaborative Plan of Care with patient, family, and the interdisciplinary trauma team via trauma team conferences; coordinates plan for patient teaching and discharge planning on all trauma and acute stroke patients.
Responds to trauma team and stroke team activations in the hospital when available.
Makes in-house rounds on trauma and acute stroke patients and evaluates performance measure compliance.
Relays follow-up patient information to referring pre-hospital, physicians, and hospitals via phone calls and trauma letters.
Develops policies/protocols/guidelines/standing orders based on benchmarks for standards of trauma care
Provides patient care by:
Intervening at critical and/or key points to achieve expected outcomes.
Utilizes the nursing process in providing patient care.
Plans and selects appropriate interventions for patient care that will result in the desired outcome, with consideration for appropriate utilization of human and material resources.
Assures competency of equipment, procedures, assessment, and monitors utilized in the delivery of trauma care.
Orients and educates medical staff, nursing staff, students, and general public to Trauma Systems and stroke care/protocol.
Participates in orientation of new staffâ"to include locum tenem Emergency Department physicians.
I! nforms the importance of Trauma Systems with staff and students as appr! opriate.
Functions as a role model and resource person.
Participates in peer review.
Identifies educational opportunities for medical staff, nursing staff, and ancillary departments regarding current trends in trauma care and acute stroke care.
Participates in legislative issues that impact trauma care in the region and state.
Attends continuing education pertinent to delivery of trauma care and acute stroke care.
Establishes a network for contacts and a support system from other trauma nurse coordinators and stroke care coordinators.
Develops an outreach program to the region for education and support in developing trauma systems and acute stroke care and delivery of quality trauma care and acute stroke care.
Develops a prevention program in the community and region that targets high-risk behavior and trends of injury. Provides education and screening events for the local community and serves as a comm! unity resource.
Coordinates monthly trauma conferences, trauma M&M, and trauma performance improvement meetings (includes clerical duties such as room reservation, memos, minutes, etc.) Chairs facility stroke steering committee.
Completes annual performance appraisal for staff. Provides feedback to Director regarding individual staff performance/development. Counsels and disciplines staff as warranted. Monitors/develops department Policies and procedures in collaboration with professional staff for the assigned areas; writing, implementing and achieving goals. Responsible for planning/conducting staff meetings, maintaining standards of practice and performance set by hospital, unit and regulatory agencies.
Participates in Performance Improvement activities.
Contributes to quality of patient care through Performance Improvement activities, Performance Improvement audits, and cost/benefit analysis.
Collects trauma data with entry i! nto trauma registry, and collects statistical analysis. Analyzes data r! elated to stroke care.
Concurrent and retrospective monitoring of trauma patients for clinical and system indicators of variance as set by American College of Surgeons and our own Trauma Serviceâs performance improvement plan.
Prepares Performance Improvement Plan on quarterly and prn basis for presentation to administration and appropriate medical committees.
Integrates educational needs as identified in Performance Improvement activities.
Improves the referral process as needed.
Attends and participates in approved/requested committees and meetings.
Participates in Emergency Care Network (ECN).
Attends Continuing Education Medical Informatics (CEMI) to identify potential CME topics from a trauma perspective.
Appointed member of the State of Nebraska Trauma Development Board to develop a State trauma system per legislation.
Coordinates activities related to the facilities Emergency Operation! s Plan, serving on the Safety Committee as the chair of the Emergency Management program, and provides guidance and education related to the development and implementation of the Emergency Operations plan and the related regulations.
Assumes budget responsibility in developing capital, operational and personnel budgets. Responsible for the formulation of budgets for assigned areas, promoting optimum allocation and utilization of resources across the division to accomplish defined goals. Plans and suggests equipment and supply needs to provide appropriate patient care.
Maintains knowledge of current trends and developments in the field by reading appropriate books, journals and other literature and attending related seminars, conferences and the like.
Qualifications
Education:
BSN required, Masters degree preferred.
Experience:
Minimum of three years on-the-job experience in Emergency Department or Critical C! are area within the last five years.
Knowledge of trauma syste! ms from either pre-hospital or in-house perspective, including critical care.
License/Certification:
Current Nebraska license as a Registered Nurse without restrictions
ACLS certified.
Certification in specialty or closely related field desired
