Part time opening, working two 12-hour shifts per week on a rotating schedule, including a combination of day and night shifts and two weekends every six weeks. As a Registered Nurse, assesses, plans, implements and evaluates care for all patients including on the Medical-Surgical, Maternal-Child, Emergency Department units. The RN in collaboration with the assigned nursing assistant or Emergency Room Technician will direct and provide person-centered care. The RN will use evidenced based practice standards, polices and procedures and current national professional organizations standards and/or position statements to guide professional practice and to provide consistent, safe and quality care to each patient/family. The RN will develop an individualized plan of care in collaboration with the patient/family, health care provider and other involved department staff to attain expected outcomes. The RN will coordinate and document the implementation of the individualized plan of care and progress to expected outcomes. Assessment: Collects comprehensive data pertinent to the patient’s health and current situation: Collects data in a systematic and ongoing process. Involves the patient, family and other healthcare providers as appropriate in holistic data collection. Prioritizes data collection activities based on the patient’s immediate condition, or anticipated needs of the patient or situation. Documents relevant data legibly and accurately. Diagnosis: Analyzes the assessment data to determine the diagnoses or issues: Determines the nursing diagnoses or issues based on the assessment data. Validates the diagnoses or issues with the patient, family, and other healthcare providers when possible and appropriate. Documents diagnoses or issues in a manner that facilitates the determination of the expected outcomes and plan. Outcomes identification: Identifies expected outcomes for a plan individualized to the patient or the situation. Involves the patient, family, and other healthcare providers in formulating expected outcomes when possible and appropriate. Derives culturally appropriate expected outcomes from the nursing diagnoses Defines expected outcomes in terms of the patient, patient values, ethical considerations, and the environment. Develops and modifies expected outcomes that provides direction and estimated timeline for the continuity of care. Documents accurately and legible expected outcomes as measurable goals. Planning: Develops a plan that prescribes strategies and alternatives to attain expected outcomes: Develops an individualized plan considering patient characteristics or the situation (age and culturally appropriate, environmentally sensitive) Develops the plan, the priorities, timeline in collaboration with the patient, family, other health care providers and others as appropriate. Uses the plan to provide direction to other members of the healthcare team. Documents legibly and accurately the plan of care. Health teaching and Health Promotion: Employs strategies to promote health and a safe environment: Provides health teaching that addresses such topics as healthy lifestyles, risk-reducing behaviors, developmental needs, activities of daily living and preventive self-care. Uses health promotion and health teaching methods appropriate to the situation and the patient’s developmental level, learning needs, readiness, ability to learn, language preference, and culture. Seeks opportunities for feedback and evaluation of the effectiveness of the strategies used. Documents accurately and legibly health teaching and promotion.

Evaluation: Evaluates progress toward attainment of outcomes:
Evaluates the effectiveness of the planned strategies in relation to patient responses and the attainment of the expected outcomes. Includes the patient, family, health care providers and any others involved in the care or situation in the evaluation process. Uses ongoing assessment data to revise the nursing diagnoses, outcomes, the plan and the implementation as needed. Documents accurately and legibly progress to outcomes. Quality of Practice: Systematically enhances the quality and effectiveness of nursing practice: Demonstrates quality by documenting the application of the nursing process in a responsible, accountable, and ethical manner.
